We're full service

We work closely with both the patient and the physician to determine the exact level of home health care needed. The care provided may be temporary, consisting of a few days a week for a few short weeks, or it may be essential to be seen daily for a couple of months. Whatever your needs, we will provide the care that is necessary to restore optimum health.

Southland Home Health believes that home care should be more than just nursing attention. We believe that in order to provide the highest quality of care, we must treat our patients like our own family. We select only nursing staff who have strong backgrounds in providing reliable care. We have ongoing training for our nurses and aides to ensure they see patients as more than just medical situations; our patients become our first priority and are as important to us as our own family.

Southland Home Health provides quality, full service health care to patients and clients in the comfort of their homes. We provide comprehensive, multidisciplinary in-home health care services to promote, maintain and restore health and to minimize the effects of illness and disability. Southland Home Health’s team of knowledgeable professionals is dedicated to excellent, compassionate care for patients, clients and their families.

Southland Home Health is a home health agency who prides themselves on giving our patients the highest quality of home care. Our home health care professionals include RN’s, LPN’s, CNA’s, and home health aides, who provide care to those desiring to remain living at home as well as those who are in need of wound care and skilled nursing.

Our Home Health Disciplines Include:


We provide a full range of nursing services including wound care, IV therapy, diabetic care, catheter care, cardiac and respiratory assessment, medication management, pain management, and cancer care.


Our Physical Therapist work to restore the mobility and strength through the use of exercise, massage, and other methods in helping relieve pain from a physical injury or a disability.


Nurse Aides provide assistance with daily living activities and personal care.


Our MSW’s help with social and emotional concerns related to an illness.This includes counseling or help finding resources within the community.


Our psych nurses approach the care of a patient with mental illness from a holistic perspective, within his/her chosen environment, we will be successful in preventing frequent hospitalizations, thereby, making psychiatric home health care the most cost-effective treatment available for this patient population.


Our OT’s help to return patients to their usual activities of daily living (ADL’s) such as bathing, meal preparation and housekeeping after an illness.


Our ST’s help assist with problems involving speech, language and swallowing.

We Provide Personal Care, Management and Treatment of the Following:

Disease Management

Support and education to patients with chronic illnesses including, CHF, COPD, CAD(Heart Disease), Stroke, Hypertension, Diabetes, Ostomy, Enteral and Respiratory with the ultimate goal of avoiding unnecessary rehospitalizations and ER visits.

Medication Management

A multidimensional process of coordinating, monitoring, and assessing medications to assure patient compliance with a specific medication regimen while ensuring the patient avoids potentially dangerous drug interactions and other complications.

Anodyne Therapy

Provides significant symptomatic relief through a non-invasive medical device. The system emits monochromatic infrared light (MIRE), providing an effective treatment for pain, poor circulation and muscle spasm caused by a range of factors including acute injuries, chemotherapy, diabetes and vascular disease.

IV Therapy

Administration and monitoring of intravenous infusions of fluids and medications.

Diabetes Management

The management of blood glucose levels through teaching interventions with patients and their families.

Cardiovascular Programs

Various programs to help patients who are recovering from cardiac related diagnostic procedures or surgeries. Programs include education, cardiac monitoring and personal care.

Orthopedic Programs

Individualized programs to aid in recovery after an orthopedic surgery including orthopedic injuries, joint replacements and complex orthopedic diseases.

Wound Care

The prevention of wound complications and promotion of wound healing.
